This is a tamper-evident laser-printable product/mailer created from a special 1-part form paper or stationery.
It is used by many institutions, including banks & internet service providers, to issue PINs and login secrets or sensitive information to individuals in a highly secure format.
The PIN or data is printed directly onto the top layer of the stationery or tab, attached to a carrier sheet. The background of the tab disguises the PIN so that it cannot be read with the naked eye without pulling the tab.
